Registration for the five day Symposium can be done two ways:

  1. By mail and US $ check. Download the STD6 registration form, fill it out and mail it back with the check covering the applicable registration fees.
  2. With a credit card and fax. Download the STD6 registration form, fill it out and fax it back to (831) 459 3422, attention Jonathan Miller.
